These fields are what Garmin Pilot and ForeFlight show at the top of the checklist, and what the printed cards carry as a title.
Your numbers
Fill these in for the airplane you fly and they flow into every download. Leave one blank and the EFB files keep the general wording, while the printed cards leave you a box to write in.

[carb] Carb heat ~ ON | Only appears when that configuration is selected above. A fuel-injected engine has no carb heat, so the line is dropped rather than left as noise. |
Write airplane-specific numbers either as a fill-in field —
[[VY]] — or as the concept, VA OR BELOW,
POH SETTING. Never as a bare blank: you cannot fill in an
EFB checklist with a Sharpie, and a checklist full of ___
is worse than useless in flight. A fill-in field is safe because it
always falls back to real wording in the EFB files, and only becomes
an empty box on paper.

Both open your browser's print dialog. Choose Save as PDF as the destination if you want a file rather than paper. The colours print either way — there is no background-graphics setting to remember.
.fmd is not a published format. This tool writes it from
community reverse-engineering, and ForeFlight has neither published nor
blessed it. Check the import on the ground before you rely on it in the
air, and use the plain-text download if it does not work for you.
